We work alongside physicians, nurse practitioners, hospice and palliative care teams, home health agencies, pharmacies, hospital discharge planners, and case managers. Our goal is simple: make it easy to coordinate care for the people we both serve.
Referrals
If you have a patient who may be a fit for a small, licensed Adult Family Care Home in Lake County, call us at (352) 404-6699. We can usually tell you within a day whether we have availability and whether the level of care is appropriate.
What we need for admission
- AHCA Form 1823, Resident Health Assessment, completed within 30 days before admission
- Current medication list and orders
- Relevant history, diagnoses, and any orders for hospice, home health, or therapy
- Advance directives and DNRO, if applicable
The Real Care Provider Portal
Approved providers get secure access to our portal, where you can:
- Upload health assessments, orders, medication lists, labs, and discharge summaries directly to a resident's record
- Message the administrator securely about a specific resident
- See a confirmation when we've reviewed what you sent
Access is limited to the residents you're involved with, and every action is logged. Please do not send patient information through the general contact form or by unsecured email.
