Our commitment
Real Care Adult Family Care Home keeps your health information private as required by federal and Florida law. We will not use or share your information other than as described here unless you tell us in writing that we may.
How we may use and share your information
- For your care: with your physician, pharmacy, hospice, home health, therapists, and hospital or emergency personnel.
- For payment: to bill you or your representative, or an insurer you authorize.
- To run our home: for quality review, staff training, and regulatory compliance.
- With your family or representative: with people involved in your care, unless you object.
- As required by law: including reports to the Florida Agency for Health Care Administration, Adult Protective Services, public health authorities, law enforcement, or a court order.
- In an emergency: to protect your health or safety.
Your rights
You have the right to see and get a copy of your records; to ask us to correct them; to ask us to limit what we share; to ask us to communicate with you in a certain way or place; to get a list of certain disclosures we've made; to receive a paper copy of this notice; and to file a complaint with us or with the U.S. Department of Health and Human Services without fear of retaliation.

We may update this notice. The current version is posted in our home, on our website, and in our portal.
